Conditions of Entry: Cavity Free Club Quarterly Prize Draw
Who is eligible?
1.1 Children aged 1-12 will be eligible to join the Cavity Free Kids Clubhouse and adolescents aged 13-17 to join the Cavity Free Youth Club, providing they meet entry criteria.
How to enter:
2.1 At the dental exam visit, if a child/adolescent is deemed caries free, they will be entered into our quarterly prize draw.
2.2 An entry ticket will be given once thorough examination, clean and radiograph images (if relevant) are taken and patient is deemed cavity free. Details will be recorded on a raffle ticket and placed into the prize bowl, where a ticket from each cavity free Club will be drawn every 3 months.
2.3 Entry tickets will only be valid for one draw cycle, after which entries will be discarded.
Conditions of entry:
3.1 – Patient must attend six monthly visits and:
– Members must have regular radiographs
– At every recall, members must be deemed cavity free
– Follow simple Club rules- brush 2 x daily, floss, have tooth friendly diet – limit sweets!
3.2 At any visit you have a cavity you will not be eligible for prize draw.
3.3 At subsequent visits, if cavity free, you will re-join quarterly prize draw.
3.4 There is no entry fee or no purchase necessary to enter this competition.
Start and Closing Dates:
4.1 Prize draws will occur quarterly.
When the prize will be drawn:
5.1 Prize draw will be conducted on the 1st working day of the next month by Dentalspa staff, at the Dentalspa Geelong practice (20 Little Ryrie Street, Geelong). There will be two winners drawn- one from each Cavity free club.
1st April, 1st July, 1st October and 1st January
5.2 No cash alternative to the prizes will be offered.
5.3 Prizes are not transferrable.
Details and values of prizes:
6.1 Prizes will vary each competition cycle, but will be a gift voucher to the sum of $20.
6.2 Gift vouchers to be determined at each competition cycle by Dentalspa Geelong and may be subject to change. Vouchers may include- itunes, movie, sports girl, EB games, Coles Myer, Big W, movie tickets etc.
6.3 Dental Spa’s Cavity Free Kid’s Clubhouse- 1 – 12yo- $20
6.4 Dental Spa’s Cavity Free Youth Club- 13 – 17yo- $20
Notification of winner:
7.1 Winners will be selected at random from all entries received.
7.2 Winners names are final and no correspondence will be entered into.
7.3 Winners will be notified within a week of prize draw and advised when and where the prize can be collected.
7.4 By entering the competition, entrant is indicating his/her agreement to be bound by these terms and conditions and consent received by parent/guardian.
7.5 Winner will be announced through website blogging and through our Facebook page. If you do not wish to have their details published, a generic ‘boy/girl aged 6 from Highton’ format will be used. Parental consent is required:
